AP Center is a guided exercise facility for people with neurological conditions who have completed formal rehabilitation. Our role is maintenance and continuity, not clinical rehabilitation.
Why continuity matters
Function achieved in rehabilitation is not self-sustaining. Once structured activity stops, many discharged patients lose ground within months. AP Center offers a regular, supervised setting where they keep working — which protects the outcome your team already achieved.

Our position in the care chain
AP Center deliberately stays out of clinical rehabilitation. We do not diagnose, we do not set rehabilitation goals, and we do not replace therapy. That boundary is what makes rehabilitation centres referral partners rather than competitors.
Sessions are led by trainers under a physical therapist who assesses each client and monitors progress. We are happy to align on individual patients where that is useful.
Who to refer
- Able to sit in a wheelchair.
- Able to understand and follow instruction.
- Able to communicate pain or distress.
- No open pressure wounds.
- Readiness confirmed by our physical therapist at intake.
A referral or request starts an intake conversation; acceptance is decided by our physical therapist. Function level itself is not a barrier — people with a wide range of ability take part.
